Did I lose my Gamertag forever?

I have this very important gamertag attached to a hard drive that no longer works (It gives me E68 whenever I try and boot my console up with it in, but works fine with a USB) and there is a very important gamertag to me on there. I have NO information about this account - no e-mail, no nothing, but I can assure you I was the one who created it.

Is this gamertag now lost forever? Cheers for any replies.
